Fees

The yearly dues for residents of Japan are 10,000 yen for Clinical Members and 6,000 yen for Associate Members. For non-residents of Japan the yearly dues are 5,000 yen for Clinical Members and 3,000 yen for Associate Members. The dues are due and payable on January 1 of each year for all members, including new members who have joined during the previous year.

The Application Process

Please carefully read and follow the instructions below. After you have submitted your application form and supporting documents your application will be considered by the Membership Secretary and others designated by the board. Appeals on membership decisions can be made to the full board on the applicant's request as outlined in the IMHPJ Constitution May 2000.

Public Information and Confidential Information

If you are accepted as a member, all Public Information will be included in the printed Membership List that is publicly distributed and is available online at the IMHPJ website. All Confidential Information will be distributed solely within IMHPJ.

How to Apply

Please submit the following.

Two Application Forms:

• Complete the On-line Information Form available on this website and submit it online.
• Print out the  Confidential Application Form (PDF) also available on this website, fill in the information, and mail the form to the Membership Secretary, Toshiko Watanabe:

Please sign this form to indicate that you have read the IMHPJ Code of Ethics and agree to abide by it. The Code of Ethics is available online in the "About IMHPJ" section of this site.

Documentation: Please mail

1. All copies of licenses, professional association memberships and degrees.
2. Criteria set out for any licenses you hold.
3. Documentation about how you meet the clinical membership supervision requirements.
4. How your academic training meets the clinical membership criteria.

In order to be listed in IMHPJ public information, such as directories or the web site, documentation of licenses, degrees, training, and professional organization membership must be provided. Any current licenses should be updated to the membership secretary annually.

Photo and Map:
To upload personal photos and location maps to the site - just click the "browse" button on the application form, find your graphic/digital files (.gif or .jpg/.jpeg) and upload it. If you you do not have photos in file format i.e. you only have a hard copy you will need to scan it and then forward it to the web site coordinator so that it can be edited for size and then uploaded (Web site Coordinator, Reggie Pawle pawle@mbox.kyoto-inet.or.jp)
